This notice explains how West Street Pharmacy uses the information you give us through this website, in particular the details you submit on our “Join Us” nomination form.

When you complete the nomination form we collect your name, date of birth, postcode and phone number, and, if you choose to give them, your address and email address.
We use these details only to process your request to nominate us as your regular pharmacy and to contact you about that request. We do this on the basis of the consent you give by ticking the box on the form.
The form sends your details to our pharmacy inbox by email through a form-handling service. We do not use your information for marketing and we never sell it.
We keep the details from your form only for as long as we need them to process your nomination and to meet our record-keeping responsibilities, after which they are securely deleted.
We don’t share your information with anyone else except where it’s necessary to provide the service you’ve asked for, or where we’re required to by law.
You can ask us for a copy of the information we hold about you, ask us to correct or delete it, and withdraw your consent at any time. To do any of these, just contact us using the phone number or email above.
If you’re unhappy with how we’ve handled your information, you can contact us, or complain to the Information Commissioner’s Office (ICO) at ico.org.uk.
